MEDICAL INFORMATION
Medical or Mental Health Condition:
If you are given a delayed confinement reporting date by the court and you suffer from any chronic, any acute physical, or any psychological ailment, or you are taking any medication, whether prescribed or over the counter, YOU MUST make arrangements to see the jail nurse at least five (5) business days prior to our reporting date. This is critical in order for us to provide you with the care you have to have while incarcerated.
